“Internal Mammary Node (IMN) Spotlight: Does Therapeutic Irradiation Steal the Show?
Target objective
Explore if IMN radiotherapy dose affects relapse (IMNR) and OS in breast cancer.
Methods
Case-control study (70 IMNR cases vs. 140 controls), BC Cancer cohort 2005-14, IMN doses calculated from CT (EQD2), ≥40 Gy as therapeutic, multivariable Fine-Gray and Cox analysis.
Results
- radiotherapy ≥40 Gy low IMNR (HR=0.36, p=0.009) and high OS (HR=0.49, p=0.02)
- Mastectomy (HR=2.11, p=0.02), larger tumors, and N+ high risk IMNR.
Clinical impact
IMN radiotherapy with therapeutic dose (≥40 Gy) is critical for controlling IMN and high OS in high-risk cases!”
