“Access to healthcare can be a powerful determinant of patient outcomes.

This large cohort study published in JCO Global Oncology investigates the impact of treatment funding source on survival in patients with bladder cancer.

Among 6,217 patients:

5-year OS in privately funded patients: 68.5%
5-year OS in publicly funded patients: 53.7%

The survival gap persisted independently of age, TNM stage, or education level. Funding source remained an independent predictor of overall survival (aHR: 1.41; p < .001).

These findings carry broader implications beyond Brazil: Structural disparities in healthcare access can decisively shape clinical outcomes. Yet these gaps are not inevitable — with early diagnosis, equitable resource allocation, and policy reform, they can be closed.”
