Despite surgical advances, patient selection remains the weak link in GI oncology. We’re still far from optimally identifying who truly benefits from surgery. Better biomarkers (ctDNA, CTCs, CA19-9, biology time tests) are urgently needed. Precision in decision-making must match the precision of modern procedures. A timely reminder from Barcelona: let’s cut less, think more.”
